NEW PATIENT PAPERWORK:
This is to be filled out before your new patient appointment. This applies if you have never been in our office before or if you have not been in to see us in over 3 years.
UPDATE PATIENT PAPERWORK:
This is to be filled out before your appointment if you have had any insurance or personal information changes since you’ve last been in or if you have had a sizeable gap in treating at our office.
SPORTS PHYSICAL PAPERWORK:
This is to be filled out before your sports physical appointment. The second page also has to be signed by the parent/guardian before the student can be seen. The third and fourth page are for the Doctor to fill out.
AUTO PERSONAL INJURY PAPERWORK:
This is to be filled out before your appointment if you have been in a car accident.
CHUSA PAPERWORK:
This is to be filled out before your appointment if you are interested in joining Chiro Health USA as a discounted self-pay option.
